Participating providers are subject to change. For verification, contact Bluegrass Family Health (BFH). Your participating provider's agreement for providing covered services may include financial incentives or risk sharing relationships related to provision of services or referrals to other providers. These incentives and risk sharing relationships are for the purpose of controlling the cost of health care and are not intended directly or indirectly to limit or reduce any medically necessary services for you or your family. If you have any questions regarding such incentives or risk sharing relationships, please contact your provider or BFH. A participating provider may not, under any circumstance, bill, charge, collect a deposit, seek compensation, remuneration, or reimbursement from, or have any recourse against BFH members for services provided in accordance with the provider agreement. This provision does not prohibit collection of deductible amounts, copayment amounts, coinsurance amounts, and amounts for noncovered services. Any provider that meets the enrollment criteria and is willing to meet the terms and conditions for participation has the right to become a provider for Bluegrass Family Health. Pursuant to applicable state law, providers do not have to participate in all products. Please note that some providers are listed as participating on certain products only. It is possible that you may receive services from a non-participating provider (such as anesthesiology, pathology, radiology) during a visit to a contracting provider. Coverage is determined by your specific benefit plan and may not be accurately determined based on the providers listed within this directory.
